Health Forms

For information regarding the health forms, please contact our school nurses, Mrs. Kieran Mitchell, RN and Mrs. Christine Danaher, RN at nurse@bishopireton.org during the summer or 703-212-5166 after August 10.

Forms: Each new student provides a school physical and immunization record before the first day of school. If the student wants to play any sport during the year, a second physical - the athletic physical - must be submitted before any tryouts. These forms are scanned and submitted online via the secure Magnus Health system. Do not drop off paper forms to the office or give to coaches. No tryouts can occur without a valid Virginia High School League athletic physical uploaded into Magnus.

Magnus Health is easily accessed through the My BI Portal with no second sign in or password to remember. Just sign into the Portal, click on the “Resource” page and click on orange M for Magnus square. This opens your student’s “to do” list. You will receive an email in early July from Magnus when your account is ready for uploads. The conditional questions that you must complete in Magnus will identify all the forms that you are required to upload for your student.

Medication: If your student needs medication during the school day, including an inhaler or EpiPen, there are medication forms to have your student’s doctor complete and submit through Magnus Health. It is recommended that these forms be completed at the physical appointment and then scanned and submitted following that appointment.

If your student needs physical accommodations, medication or any treatments during the school day, please contact the School Nurses by August 10.

Online consents and permissions: Emergency Care permission, emergency contacts, media release, health history and other consents and permissions are electronically signed by a parent or scanned/uploaded into your student’s secure Magnus Health account beginning July 1. The due date for these consents and scanned forms is August 1 to allow time for review.
